\u0130lk uygulama \u0130ngilterede yap\u0131lm\u0131\u015f ve sonu\u00e7 olarak ba\u015far\u0131 sa\u011flam\u0131\u015ft\u0131r.\u00a0 T\u00fcp bebek uygulamalar\u0131nda elde edilecek ba\u015far\u0131 oran\u0131n\u0131 artt\u0131rabilmek i\u00e7in, \u0130ngiltere\u2019nin Cambridge \u00fcniversitesinde geli\u015ftirilen t\u00fcm kromozom analizi (24 sure) y\u00f6ntemi ile d\u00f6llenmesi sa\u011flanan yumurtadan al\u0131nan h\u00fccredeki b\u00fct\u00fcn kromozomlar\u0131n, k\u0131sa s\u00fcre i\u00e7erisinde incelenmesi sa\u011flanmaktad\u0131r. Bu y\u00f6ntem ile beraber, daha \u00f6nce yap\u0131lm\u0131\u015f olan t\u00fcp bebek uygulamalar\u0131na g\u00f6re daha y\u00fcksek ba\u015far\u0131 oranlar\u0131 sa\u011flanmaktad\u0131r. D\u00fc\u015f\u00fck durumu ile de \u00e7ok az kar\u015f\u0131la\u015f\u0131lmaktad\u0131r. D\u00fc\u015f\u00fcklerin en \u00f6nemli meydana gelme sebebi ise, kromozomal kaynakl\u0131 anormalliklerdir. Bu y\u00f6ntem sayesinde b\u00fct\u00fcn kromozomlar\u0131n say\u0131s\u0131n\u0131n normal de\u011ferlerde oldu\u011fundan emin olunup, daha sonras\u0131nda anne aday\u0131n\u0131n rahmine transferi sa\u011flanmaktad\u0131r. Bu teknik, T\u00fcrkiye\u2019de \u015fuan i\u00e7in daha yeni yeni uygulamaya giren bir y\u00f6ntemdir.<\/p>\n

2 ) T\u00fcm kromozom testi ne kadar s\u00fcrede yap\u0131l\u0131r?<\/p>\n

T\u00fcm kromozom testi, sadece 12 saat gibi k\u0131sa bir s\u00fcre i\u00e7erisinde 24 kromozom say\u0131sal normallik a\u015f\u0131s\u0131 arac\u0131l\u0131\u011f\u0131 ile taranmaktad\u0131r. Anoploidi, yani kromozom say\u0131lar\u0131nda herhangi bir anormallik tespit edilmez ise, o zaman t\u00fcp bebek tedavisine devam edilir. Bu y\u00f6ntem ile daha \u00e7ok ya\u015f\u0131 b\u00fcy\u00fck olan kad\u0131nlarda meydana gelen hamileliklerde, kromozomal bozukluklara \u00e7ok fazla rastlanmaktad\u0131r.<\/p>\n

3 ) Kapsaml\u0131 kromozom taramas\u0131 ile beraber b\u00fct\u00fcn kromozomlar mercek alt\u0131nda<\/p>\n

Kapsaml\u0131 Kromozom Taramas\u0131 yakla\u015f\u0131m\u0131 t\u00fcp bebek uygulamalar\u0131n\u0131n yap\u0131lmas\u0131 esnas\u0131nda, hamilelik oran\u0131nda art\u0131\u015f elde edebilmek amac\u0131 ile mevcut olan en son teknolojinin ve bilgi d\u00fczeyinin kullan\u0131ld\u0131\u011f\u0131 bir yakla\u015f\u0131m olmas\u0131 sebebi ile son derece \u00f6nemli bir yere sahiptir. Zira bu yakla\u015f\u0131m esnas\u0131nda, ilk olarak tedavi yapt\u0131rmak isteyen e\u015flerin embriyolar\u0131, m\u00fcmk\u00fcn olan en son a\u015famaya kadar b\u00fcy\u00fct\u00fclmesi sa\u011flan\u0131r. Embriyolar\u0131n anne rahmine tutunabilme ihtimali en y\u00fcksek olarak tespit edilen embriyolara aCGH metodu (array Comparative Genomic Hybridization) uygulanarak, t\u00fcm kromozomlar\u0131n\u0131n (24 kromozom) analiz edilebilmesi i\u00e7in biyopsi yap\u0131lmas\u0131na ihtiya\u00e7 duyulur ve bu embriyolar dondurulmas\u0131 sa\u011flan\u0131r.<\/p>\n

\u0130\u015flemin yap\u0131ld\u0131\u011f\u0131 ay veya birka\u00e7 ay i\u00e7erisinde, ortaya \u00e7\u0131kan genetik analiz sonucuna g\u00f6re normal olarak kabul edilen embriyolar \u00e7\u00f6z\u00fclerek, do\u011fal bir rahim ortam\u0131na nakil edilmesi ger\u00e7ekle\u015ftirilir. Bu uygulama s\u0131ras\u0131nda y\u00fcksek oranda ba\u015far\u0131l\u0131 sa\u011flanabilmesi i\u00e7in, embriyo biyopsi i\u015flemi, genetik inceleme ve embriyo dondurma\/\u00e7\u00f6zme uygulamalar\u0131n\u0131n son derece programl\u0131 ve y\u00fcksek standartta uygulanmas\u0131 gerekir. B\u00f6ylelikle, son derece titiz bir tak\u0131m \u00e7al\u0131\u015fmas\u0131na da ihtiya\u00e7 duyulur.<\/p>\n

Yurtd\u0131\u015f\u0131nda se\u00e7kin birka\u00e7 klinik taraf\u0131ndan ger\u00e7ekle\u015ftirilen ve \u00f6nce ki y\u0131llarda Amerikan \u00dcreme T\u0131bb\u0131 Derne\u011fi\u2019nin y\u0131ll\u0131k toplant\u0131s\u0131nda \u00f6ne \u00e7\u0131kar\u0131lan \u00e7al\u0131\u015fmalarda, daha \u00e7ok ya\u015f\u0131 b\u00fcy\u00fck olan anne aday\u0131 olgular\u0131nda, KKT yakla\u015f\u0131m\u0131 ile beraber sa\u011flanan embriyolarla, embriyo transferi uygulamalar\u0131nda kad\u0131n ya\u015f\u0131n\u0131n gen\u00e7 oldu\u011fu olgular kadar y\u00fcksek hamilelik oranlar\u0131n\u0131n sa\u011fland\u0131\u011f\u0131 da ifade edilmektedir.<\/p>\n

\u0130ncelenen t\u00fcm kromozomlar y\u00f6n\u00fcnden normal bulunan bir embriyo her zaman gebelik olu\u015fturur mu?<\/p>\n

Kromozomal bozukluk nedeni ile gebelik olu\u015fmamas\u0131, veya olu\u015fan gebeli\u011fin erken d\u00f6nemlerde kaybedilmesi riski her ya\u015fta ya\u015fanabilecek bir durum. Fakat kad\u0131n ya\u015f\u0131n\u0131n \u00f6zellikle 35\u2019in \u00fczerinde oldu\u011fu \u00e7iftlerde \u00f6zellikle ciddi olarak artmakta. Bu nedenle genetik ay\u0131klama ile normal embriyolar\u0131n se\u00e7ilmesi bu riski belirgin oranda azalt\u0131yor. Di\u011fer taraftan gebelik olu\u015fmas\u0131 i\u00e7in sadece genetik olarak normal bir embriyo elde etmek yeterli de\u011fil. Sa\u011fl\u0131kl\u0131 bir rahim dokusu da gebelik olu\u015fmas\u0131 i\u00e7in olmazsa olmaz bir kriter.<\/p>\n

Gerek PGT, gerekse kapsaml\u0131 kromozom taramas\u0131 uygulamas\u0131nda incelenebilecek embriyo say\u0131s\u0131n\u0131n artt\u0131r\u0131lmas\u0131 amac\u0131 ile baz\u0131 hastalarda y\u00fcksek doz ila\u00e7 kullanabilmekteyiz. Her ne kadar k\u0131smen amaca ula\u015f\u0131l\u0131yor gibi g\u00f6r\u00fcnse de asl\u0131nda kullan\u0131lan y\u00fcksek doz ila\u00e7lar\u0131n embriyonun tutunaca\u011f\u0131 rahim i\u00e7i duvar\u0131na negatif bir etkisi var ve bu durumda baz\u0131 hastalarda genetik olarak normal bir embriyo elde edilebilse dahi uygun olmayan bir rahim dokusu nedeni ile gebelik elde edilemeyebiliyor. Bu durumda \u015fans\u0131 artt\u0131rabilmek i\u00e7in genetik i\u015flem yap\u0131lan embriyolar dondurularak saklanabiliyor ve tedavi sonras\u0131 v\u00fccut 1-2 ay dinlendikten sonra embriyolar \u00e7\u00f6z\u00fclerek do\u011fal bir rahim ortam\u0131na naklediliyor.
