T\u00fcp bebek tedavisinde \u00f6nemli i\u015flemlerden biri de embriyo zar\u0131n\u0131n inceltilmesi i\u015flemidir.<\/p>\n

Cerrahi sperm arama (PESA, PTSA, TESE)<\/p>\n

Erke\u011fin menisinde hi\u00e7 sperm olmamas\u0131 durumunda (azospermi) mikroenjeksiyon i\u015fleminde kullan\u0131lacak olan spermin testislerden al\u0131nmas\u0131 g\u00fcndeme gelmektedir. Bu uygulaman\u0131n ba\u015flamas\u0131 ile erkek k\u0131s\u0131rl\u0131\u011f\u0131 konusunda devrim ya\u015fanm\u0131\u015ft\u0131r. T\u0131kan\u0131kl\u0131\u011fa ba\u011fl\u0131 azospermi olgular\u0131nda kanallar\u0131n i\u00e7ine ince bir i\u011fne ile girilerek sperm aran\u0131r (PESA). Bu t\u00fcr olgularda kendi klini\u011fimizde sperm bulma oran\u0131m\u0131z %99.6\u2032d\u0131r.<\/p>\n

T\u0131kanman\u0131n olmad\u0131\u011f\u0131 durumlarda i\u015fe problem daha kar\u0131\u015f\u0131kt\u0131r. Bu durumlarda erkek yumurtal\u0131\u011f\u0131n\u0131n \u00e7e\u015fitli b\u00f6l\u00fcmlerinde \u00e7ok k\u0131s\u0131tl\u0131 da olsa bir \u00fcretim s\u00f6z konusu olabilmektedir.<\/p>\n

Yumurtal\u0131\u011f\u0131n \u00e7e\u015fitli b\u00f6l\u00fcmlerinden \u00e7ok say\u0131da k\u00fc\u00e7\u00fck par\u00e7a al\u0131narak bu par\u00e7alar\u0131n i\u00e7erisinde sperm h\u00fccresi aramak gerekmektedir. Par\u00e7alar i\u011fne ile (PTSA) ya da a\u00e7\u0131k cerrahi ile al\u0131nabilir (TESE). Bu teknikle hastalar\u0131n yakla\u015f\u0131k %60`\u2019\u0131nda sperm bulunabilmektedir. \u00dcretim bozuklu\u011funa ba\u011fl\u0131 azospermi olgular\u0131nda gebelik oranlan biraz daha d\u00fc\u015f\u00fckt\u00fcr.<\/p>\n

Yard\u0131mc\u0131 \u00fcreme tekniklerine ba\u015fvuran \u00e7iftlerin yar\u0131s\u0131ndan fazlas\u0131 embriyo geli\u015fmesine ra\u011fmen gebelik olmamaktad\u0131r. D\u00f6llenme olmas\u0131na ra\u011fmen gebelik olu\u015fmamas\u0131n\u0131n kayna\u011f\u0131 muhtemelen embriyonun rahime yerle\u015fme safhas\u0131ndad\u0131r. Embriyonun rahim i\u00e7ine yerle\u015ftirilmesini takiben de\u011fi\u015fik olaylar olu\u015fmaktad\u0131r. Embriyo b\u00f6l\u00fcnmeye ve b\u00fcy\u00fcmeye devam etmekte, belli bir boya eri\u015fince kendisini \u00e7evreleyen zar\u0131 (zona pellusid) y\u0131rtarak endometriyum olarak adland\u0131r\u0131lan rahim i\u00e7indeki dokunun derinliklerine yerle\u015ferek b\u00fcy\u00fcmesine burada devam etmektedir.<\/p>\n

Gebeli\u011fin olu\u015fmamas\u0131n\u0131n en \u00f6nemli nedeni embriyonun bu zar\u0131 y\u0131rtarak d\u0131\u015far\u0131ya \u00e7\u0131kamamas\u0131 ve dolay\u0131s\u0131 ile rahim duvar\u0131na yerle\u015fememesi oldu\u011fu kabul edilmektedir.<\/p>\n

Bu problemi \u00e7\u00f6zmek i\u00e7in embriyoyu \u00e7evreleyen bu zarda transfer i\u015flemi \u00f6ncesi kimyasal veya mekanik y\u00f6ntemlerle k\u00fc\u00e7\u00fck bir delik a\u00e7\u0131larak embriyonun bu zar\u0131 y\u0131rtmas\u0131 ve rahim duvar\u0131na yerle\u015fmesi sa\u011flanmakta. Yap\u0131lan bilimsel \u00e7al\u0131\u015fmalar bu y\u00f6ntemle gebelik oranlar\u0131nda hissedilir bir y\u00fckselme oldu\u011funu g\u00f6stermektedir.<\/p>\n

Bu teknik k\u0131saca \u015fu \u015fekilde uygulanmaktad\u0131r: ilk olarak embriyo mikroskobik bir t\u00fcp yard\u0131m\u0131 ile duvar\u0131ndan emilerek sabitle\u015ftirilmekte daha sonra yine mikroskobik bir i\u011fne ile embriyo duvar\u0131ndan te\u011fet ge\u00e7ilerek iki noktada delik a\u00e7\u0131lmaktad\u0131r. Embriyo, rahim i\u00e7inde b\u00fcy\u00fcmesi devam ederken zay\u0131f olan bu noktalarda zar\u0131n\u0131 delebilmektedir.<\/p>\n

Preimpantasyon genetik tan\u0131 (PGT)<\/p>\n

Preimpantasyon Genetik Tan\u0131 (PGT), ailesinde genetik hastal\u0131klar olan \u00e7iftlerin ya da uygulanan tedavilere cevap vermemi\u015f interfil ailelerin t\u00fcp bebek y\u00f6ntemi kullan\u0131larak sa\u011fl\u0131kl\u0131 bebe\u011fe kavu\u015fmalar\u0131n\u0131 sa\u011flayan yeni bir genetik tan\u0131 y\u00f6ntemidir.<\/p>\n

Bu teknikte \u00e7iftlerden elde edilen embriyolar tek tek incelenerek genetik olarak sa\u011fl\u0131kl\u0131 olan embriyolar, anormal embriyolardan ayr\u0131l\u0131r ve anne aday\u0131na genetik olarak normal oldu\u011fu saptanan embriyolar transfer edilir. Bu sayede genetik bozuklu\u011fu olan \u00e7ocu\u011fa sahip olma riski y\u00fcksek olan \u00e7iftler i\u00e7in hamilelik en ba\u015f\u0131ndan kontrol alt\u0131na al\u0131nm\u0131\u015f olur.<\/p>\n

IVF\u2019de olumsuz sonu\u00e7lar\u0131n ba\u015fl\u0131ca sebeplerinden biri kromozom anomalisi dolay\u0131s\u0131yla meydana gelen d\u00fc\u015f\u00fcklerdir. Bu nedenle PGT, \u00f6zellikle ileri ya\u015ftaki IVF hastalar\u0131na ait oositlerde C.1 gibi y\u00fcksek oranda kromozom anomalisine rastlanmas\u0131 sebebiyle ileri ya\u015f anne adaylar\u0131na \u00f6nerilmektedir.<\/p>\n

Ayr\u0131ca \u00fclkemizde s\u0131kl\u0131kla g\u00f6r\u00fclen talasemi ve orak h\u00fccreli anemi genetik hastal\u0131klar\u0131n gebelik \u00f6ncesi analizi de PGT ile yap\u0131labilmektedir. Geli\u015fen genetik teknikler ve bilgiye ula\u015fma olanaklar\u0131n\u0131n artmas\u0131 \u00e7iftlerin, PGT ve di\u011fer prenatal tan\u0131 y\u00f6ntemleri hakk\u0131nda sa\u011fl\u0131k merkezlerine ba\u015fvurmalar\u0131n\u0131 kolayla\u015ft\u0131rm\u0131\u015ft\u0131r. As\u0131l amac\u0131 aileleri sa\u011fl\u0131kl\u0131 bebeklerine kavu\u015fturmak olan IVF, Preimpantasyon Genetik Tan\u0131\u2019n\u0131n uygulanmas\u0131 ile birlikte ba\u015far\u0131ya ula\u015fma konu\u015funda bir ad\u0131m daha at\u0131lmas\u0131n\u0131 sa\u011flam\u0131\u015ft\u0131r.<\/p>\n

Genetik tan\u0131 laboratuar\u0131 blastokist transferi
\nSon d\u00f6nemlerde geli\u015ftirilmi\u015f medium sistemleri kullan\u0131larak embryo canl\u0131l\u0131\u011f\u0131 laboratuvar ortam\u0131nda daha da uzat\u0131lm\u0131\u015f ve buna ba\u011fl\u0131 olarak g\u00fcn\u00fcm\u00fczde t\u00fcp bebek merkezlerinde, daha y\u00fcksek gebelik oranlar\u0131n\u0131n elde edildi\u011fi 5. ya da 6. g\u00fcn transferleri yayg\u0131nla\u015fmaya ba\u015flam\u0131\u015ft\u0131r. Buna blastokist transferi ad\u0131 verilir.

Embriyo dondurma<\/p>\n

\u0130nsan gametlerinin ve embriyolar\u0131n\u0131n dondurulmas\u0131n\u0131n t\u00fcp bebek prati\u011finde b\u00fcy\u00fck \u00f6nemi vard\u0131r. T\u00fcp bebek uygulamalar\u0131nda \u00e7o\u011ful gebelik riskini en aza indirmek i\u00e7in genel yakla\u015f\u0131m en fazla \u00fc\u00e7 embriyo transfer etmektir. Bu durumda akla gelen ilk soru elde edilen fazla embriyolar\u0131n ne \u015fekilde de\u011ferlendirilece\u011fidir.
\nBu \u015fekilde elde edilen fazla embriyolar\u0131n dondurulmas\u0131 hastaya hem ekonomik, hem de psikolojik bir avantaj sa\u011flamaktad\u0131r. Ayr\u0131ca dondurulan embriyolar transfer edilece\u011fi zaman hasta herhangi bir tedaviye gereksinim duymamaktad\u0131r. Embriyo dondurma i\u015flemi t\u00fcp bebek uygulamalar\u0131nda ba\u015far\u0131 \u015fans\u0131n\u0131 artt\u0131ran bir i\u015flem olarak da de\u011ferlendirilebilir.<\/p>\n

Embriyo dondurma ve \u00e7\u00f6zme i\u015flemi, embriyolar kimyasal maddelerle (kriyoprotektan) dengelendikten sonra so\u011futulmas\u0131 ve -196\u00b0C s\u0131v\u0131 nitrojen i\u00e7inde depolanmas\u0131, \u00e7\u00f6z\u00fcld\u00fckten sonra da kriyoprotektan ortam\u0131ndan uzakla\u015ft\u0131r\u0131larak ileri geli\u015fimi sa\u011flamak i\u00e7in \u00f6zel k\u00fclt\u00fcr ortamlar\u0131n\u0131n i\u00e7ine al\u0131nmas\u0131d\u0131r.
\nHer iki i\u015flem de \u00e7ok dikkatli yap\u0131lmal\u0131d\u0131r. Rutin t\u00fcp bebek ve mikroenjeksiyon uygulamalar\u0131nda embriyo dondurma ile gebelik oranlar\u0131 de\u011fi\u015fmektedir.
