Gebelik s\u00fcresince r\u00f6ntgen \u00e7ektirmek gibi radyosyona maruz kalmalar\u0131 bebekte ciddi sorunlara yol a\u00e7abilir.<\/p>\n

\u0130yonize radyasyon ve r\u00f6ntgen filmi \u0131\u015f\u0131nlar\u0131na ileri derecede y\u00fcksek dozda maruz kalman\u0131n de\u011fil gebe anne adaylar\u0131na her t\u00fcrl\u00fc canl\u0131ya b\u00fcy\u00fck zararlar\u0131 vard\u0131r. Gebelerde bu durum fetusun sa\u011fl\u0131\u011f\u0131 a\u00e7\u0131s\u0131ndan daha da risklidir. Bu nedenle hamile veya hamilelik \u015f\u00fcphesi olan ki\u015filerin r\u00f6ntgen cihazlar\u0131ndan uzak durulmas\u0131 gerekti\u011fi dikkatle ve \u00f6nemle \u00fczerinde durulan bir husustur. R\u00f6ntgen veya iyolize \u0131\u015f\u0131nlar\u0131n\u0131n verdi\u011fi zararlardan birka\u00e7\u0131 \u015f\u00f6yledir:<\/p>\n

Yukarda say\u0131lan zararlar i\u00e7in radyasyonun miktar\u0131 ve gebeli\u011fin d\u00f6nemine g\u00f6re etkile\u015fimi fark g\u00f6stermektedir. Ayr\u0131ca bu maddeler bilinen zararlard\u0131r. Radyasyonun canl\u0131lar \u00fczerinde bilinmeyen bir\u00e7ok olumsuz etkileri oldu\u011fu da bilim adamlar\u0131 taraf\u0131ndan s\u00fcrekli tart\u0131\u015f\u0131lmaktad\u0131r. Hen\u00fcz tam kan\u0131tlanmad\u0131\u011f\u0131 i\u00e7in radyasyonun zararlar\u0131 net a\u00e7\u0131klanamamakta ve bu konuda istatistiksel veri toplanmaktad\u0131r.<\/p>\n

Radyasyonun dozu \u015fayet 5 rad (5000 mrad) (0.05 Gy) alt\u0131nda bir de\u011fer de ise hen\u00fcz gebelik s\u00fcresince herhangi bir \u00f6l\u00fcm vakas\u0131, d\u00fc\u015f\u00fck, zek\u00e2 gerili\u011fi, geli\u015fme bozuklu\u011fu ya da herhangi bir anomali oldu\u011funu g\u00f6steren bir delil elde edilmemi\u015ftir. Buna g\u00f6re 5 \u2013 10 rad aras\u0131nda kan\u0131tlanmasa da bir risk oldu\u011fu bilinmekte ve bu \u00f6l\u00e7\u00fcm\u00fcn daha \u00fcst\u00fc ise y\u00fcksek risk ve tehlike olu\u015fturdu\u011fu tespiti daha net ve kan\u0131tlanm\u0131\u015f bir bilgi olarak gebelerde konjenitalmalformasyon10 rad (10000 mrad) (0.1 Gy) \u00fcst\u00fcnde dozlarda g\u00f6r\u00fclen riskli vakalar tespit edilmi\u015ftir.<\/p>\n

G\u00fcn\u00fcm\u00fczde \u00e7ekilen r\u00f6ntgenlerin \u00e7o\u011fu 5000 mrad\u0131n alt\u0131nda kalmakta ve yaln\u0131zca birka\u00e7 r\u00f6ntgeni 5000 mrad\u0131n \u00fcst\u00fcne \u00e7\u0131kmaktad\u0131r. Onlar\u0131nda en fazla 10 bin mrad\u0131 yani 0.1Gy a\u015fmamas\u0131na dikkat edilmektedir. Makalemizin sonunda hangi r\u00f6ntgen \u00e7ekimlerinin ne kadar mrad oldu\u011funu detayl\u0131 bir \u015fekilde a\u00e7\u0131klayaca\u011f\u0131z.<\/p>\n

A\u015fa\u011f\u0131da olu\u015fturdu\u011fumuz listeye g\u00f6re ancak y\u00fczlerce di\u015f filminin 5000 mrada denk gelebildi\u011fini g\u00f6rmekteyiz. Ayr\u0131ca kar\u0131n filmlerinin 20 tanesi ancak 5000 MRAD edebilmektedir. Her h\u00e2l\u00fck\u00e2rda hamile bayanlar\u0131n r\u00f6ntgen filmine maruz kalmas\u0131n\u0131 do\u011fru bulmamakla beraber ertelenebiliyorsa hamilelik sonras\u0131na ertelenmesi taraftar\u0131y\u0131z. Ertelenemeyen \u00f6zel vakalarda ise m\u00fcmk\u00fcnse hamile anne aday\u0131n\u0131n karn\u0131na kur\u015fun ge\u00e7irmez bir yelek sar\u0131lmas\u0131 ve bebe\u011fin zarar g\u00f6rmemesi i\u00e7in t\u00fcm imkanlar seferber edilerek filmin \u00e7ekilmesi gerekmektedir.<\/p>\n

Hamileli\u011fin ilk haftalar\u0131 bebek olduk\u00e7a hassas bir d\u00f6nemdedir. Hen\u00fcz embriyo var olu\u015fma a\u015famas\u0131nda oldu\u011fu i\u00e7in r\u00f6ntgen filmleri bebekte ya \u00f6ld\u00fcr\u00fcr d\u00fc\u015f\u00fc\u011fe neden olur ya da bebe\u011fe hi\u00e7bir etkisi olmaz bebek sa\u011flam do\u011far. Bu ikisinin d\u0131\u015f\u0131nda bir vaka tespit edilmemi\u015ftir. \u0130lk iki haftadan sonra ise 25 ila 26. Haftaya kadar bebek radyasyona kar\u015f\u0131 \u00e7ok dayan\u0131ks\u0131zd\u0131r. D\u00fc\u015f\u00fck veya anne karn\u0131nda \u00f6l\u00fcm, geli\u015fim bozuklu\u011fu, anomaliler ve zeka gerili\u011fi gibi s\u0131k\u0131nt\u0131lar olu\u015fmakta bu nedenle y\u00fcksek riski bu haftalarda olu\u015fmaktad\u0131r. 26. Haftadan sonra fet\u00fcs daha diren\u00e7li hale gelecek kadar b\u00fcy\u00fcd\u00fc\u011f\u00fcnden bu kadar zarar vermese de bebeklerde kansere kar\u015f\u0131 dayan\u0131ks\u0131zl\u0131k \u00f6zellikle l\u00f6semi gibi kanser vakalar\u0131 g\u00f6zlemlenmi\u015ftir. bu oranlar 1- 2 rad a kadar radyasyonlu r\u00f6ntgenler de bile g\u00f6zlemlendi\u011finden bu nedenle gebelik d\u00f6neminde anne aday\u0131n\u0131n hi\u00e7bir \u015fekilde radyasyona maruz kalmamas\u0131 gerekti\u011fi \u00fczerinde hassasiyetle durulmaktad\u0131r.<\/p>\n

Hamilelikte r\u00f6ntgen zararl\u0131 m\u0131d\u0131r?<\/strong>
\nRadyoaktif \u0131\u015f\u0131nlar olan X-\u0131\u015f\u0131nlar\u0131 ya da yayg\u0131n ad\u0131 ile r\u00f6ntgen \u0131\u015f\u0131nlar\u0131 ve bunlar kullan\u0131larak \u00e7ekilen filmler ile tomografi gibi y\u00f6ntemler v\u00fccut i\u00e7inde ya\u015fanan patolojilerin saptanmas\u0131nda son derece yararl\u0131 bilgiler veren tan\u0131 teknikleridir. T\u0131bb\u0131n hemen her alan\u0131nda zaman zaman r\u00f6ntgen filmlerine gerek duyulur.
\n\u0130yonize edici radyoaktif \u0131\u015f\u0131nlar\u0131n kullan\u0131ld\u0131\u011f\u0131 bu tekniklerin hamile bir kad\u0131n \u00fczerinde kullan\u0131lmas\u0131 do\u011fal olarak olay\u0131n i\u00e7inde yer alan her ki\u015fide endi\u015fe olu\u015fturur.<\/p>\n

T\u00fcm tan\u0131 ve tedavi y\u00f6ntemlerinde oldu\u011fu gibi r\u00f6ntgen filmlerinin de potansiyel yarar ve zararlar\u0131 mevcuttur. Bu hem hamile olan hem de olmayan ki\u015filer i\u00e7in ge\u00e7erlidir.
\n\u0130yonize radyasyon h\u0131zl\u0131 b\u00f6l\u00fcnen ve \u00e7o\u011falan h\u00fccreler \u00fczerinde daha fazla tahrip edici etkiye sahip oldu\u011fu i\u00e7in geli\u015fmekte olan fetus \u00fczerinde de zararl\u0131 etkileri olabilir. Ancak bu etkilerin doz ve s\u00fcreye ba\u011fl\u0131 oldu\u011fu unutulmamal\u0131d\u0131r.<\/p>\n

Yap\u0131lan ara\u015ft\u0131rmalarda fetusa zararl\u0131 olabilecek radyasyon dozunun 5 rad oldu\u011fu, fetusun bu miktar\u0131n alt\u0131nda radyasyona maruz kalmas\u0131 durumuda ise zarar g\u00f6rme olas\u0131l\u0131\u011f\u0131n\u0131n son derece uzak oldu\u011fu ortaya konmu\u015ftur. 5 rad hi\u00e7bir radyoloji tekni\u011fi ile ula\u015f\u0131lamayacak olduk\u00e7a y\u00fcksek bir dozdur.
\nAmerikan Aile Hekimli\u011fi Akademisi gebelik s\u0131ras\u0131nda \u00e7ekilen rontgen filmlerini g\u00fcvenli olarak s\u0131n\u0131flamaktad\u0131r. Bunun en \u00f6nemli nedeni herhangi bir tan\u0131sal rontgen fiminde fetusa ula\u015fan dozun zarar verebilecek dozdan y\u00fczlerce kez daha az olmas\u0131d\u0131r.<\/p>\n

\u00d6rne\u011fin en s\u0131k kar\u015f\u0131la\u015ft\u0131\u011f\u0131m\u0131z sorulardan biri olan hamilelikte di\u015f r\u00f6ntgeni konusuna bakt\u0131\u011f\u0131m\u0131zda a\u011f\u0131zda \u00e7ekilen tam 21 adet film netcesinde bebe\u011fe ula\u015fan radyasyon dozu anne aday\u0131n\u0131n do\u011fadan g\u00fcne\u015f \u0131\u015f\u0131nlar\u0131 vb. ile 3 g\u00fcnde ald\u0131\u011f\u0131 dozdan daha azd\u0131r. Bu kadar d\u00fc\u015f\u00fck bir dozun bebekte kal\u0131c\u0131 hasara neden olmas\u0131 ve ilerki d\u00f6nemde kansere yol a\u00e7mas\u0131 yok denecek kadar d\u00fc\u015f\u00fck bir olas\u0131l\u0131kt\u0131r.
\nBir ba\u015fka \u00f6rnek ise akci\u011fer filmidir. Hamile bir kad\u0131n akci\u011fer filmi \u00e7ektirdi\u011finde bebe\u011fe ula\u015fan radyasyon dozu ortalama 0.05 miliradd\u0131r ve bebek i\u00e7in riskli olabilecek dozdan y\u00fczlerce kez daha azd\u0131r.<\/p>\n

G\u00f6r\u00fcld\u00fc\u011f\u00fc gibi uterusa \u00e7ok yak\u0131n b\u00f6lgeler i\u00e7in \u00e7ekilen r\u00f6ntgen filmlerinde bile uterusa ula\u015fan doz zarar verebilecek olan dozun \u00e7ok daha alt\u0131ndad\u0131r.Hamilelikte r\u00f6ntgen \u0131\u015f\u0131nlar\u0131 g\u00fcvenli olarak kabul edilse bile yine de gereksiz yere \u0131\u015f\u0131n almamak i\u00e7in film \u00e7ekilirken kar\u0131n \u00fczerine kur\u015fun g\u00f6mlek konulmas\u0131 \u00f6nerilir.
\nHamilelikte r\u00f6ntgen \u00e7ekilmesi ve radyasyon?
\nE\u011fer bir kad\u0131n adet gecikmesi olmadan \u00f6nce ya da birka\u00e7 g\u00fcnl\u00fck gecikme s\u0131ras\u0131nda r\u00f6ntgen filmi \u00e7ektirirse bu durum tek ba\u015f\u0131na gebeli\u011fi sonland\u0131rmak i\u00e7in yeterli bir neden de\u011fildir. \u00c7\u00fcnk\u00fc b\u00f6yle bir durumda bebe\u011fin etkileme olas\u0131l\u0131\u011f\u0131 yok denecek kadar azd\u0131r.<\/p>\n

 <\/p>\n

Radyasyona maruz kal\u0131nan radyasyon haftas\u0131na g\u00f6re olas\u0131 etkiler \u015fu \u015fekildedir:<\/strong><\/p>\n

1. Malformasyon ve prental \u00f6l\u00fcm:<\/strong> En duyarl\u0131 olunan d\u00f6nem d\u00f6llenmeden sonraki ilk 8 g\u00fcnd\u00fcr. Bu d\u00f6nemde al\u0131nan radyasyon gebeliklerin %50-75\u2019inde d\u00fc\u015f\u00fc\u011fe neden olurken bu d\u00fc\u015f\u00fcklerin \u00e7o\u011fu beklenen adet kanamas\u0131ndan \u00f6nce meydana geldi\u011fi i\u00e7in genelde fark edilmez. \u00d6te yandan klinik olarak fark edilen gebeliklerdekid\u00fc\u015f\u00fck oran\u0131 ise %15-20 civar\u0131ndad\u0131r. \u0130nsanlarda 100 raddan daha fazla yani \u00e7ok y\u00fcksek oranda radyasyona maruz kalman\u0131n etkileri konusunda elde ver yoktur ancak hayvan deneyleri bebe\u011fin rahimde yerle\u015fmesinden \u00f6nce maruz kal\u0131nan 5-10 rad d\u00fczeyindeki radyasyonun d\u00fc\u015f\u00fck ve anomalilere neden oldu\u011funu d\u00fc\u015f\u00fcnd\u00fcrmektedir. Tan\u0131sal radyolojik incelemeler s\u0131ras\u0131nda al\u0131nan d\u00fc\u015f\u00fck doz radyasyonun bu t\u00fcr bir etkisi g\u00f6sterilememi\u015ftir.<\/p>\n

2. Geli\u015fme gerili\u011fi:<\/strong> En duyarl\u0131 olunan d\u00f6nem d\u00f6llenmeden sonraki 8-56. g\u00fcnlerdir. Japonya\u2019daki atom bombas\u0131 facias\u0131ndan sa\u011f \u00e7\u0131kanlarda yap\u0131lan incelemelerde gebeliklerinin bu d\u00f6neminde yakla\u015f\u0131k 25 rad radyasyona maruz kalanlardan do\u011fan bebeklerin daha k\u0131sa, daha hafif ve kafa \u00e7aplar\u0131n\u0131n daha k\u00fc\u00e7\u00fck oldu\u011fu saptanm\u0131\u015ft\u0131r.<\/p>\n

3. N\u00f6rolojik etkiler<\/strong>: En hassas olunan d\u00f6nem d\u00f6llenme sonras\u0131 2-15 haftalard\u0131r. Yine Atom bombas\u0131ndan sa\u011f kurtulanlarda yap\u0131lan g\u00f6zlemlerde en s\u0131k kar\u015f\u0131la\u015f\u0131lan anomalinin mikrosefali olarak adland\u0131r\u0131lan kafan\u0131n k\u00fc\u00e7\u00fck olmas\u0131 oldu\u011fu g\u00f6r\u00fclm\u00fc\u015ft\u00fcr. Mikrosefali genellikle zeka gerili\u011fi ile birlikte g\u00f6r\u00fcl\u00fcr ancak radyasyona ba\u011fl\u0131 ortaya \u00e7\u0131kan mikrosefali olgular\u0131n\u0131n sadece %25\u2019inde zeka gerili\u011fi saptanm\u0131\u015ft\u0131r. Bu hastalarda al\u0131nan her rad ba\u015f\u0131na mikrosefali g\u00f6r\u00fclme olas\u0131l\u0131\u011f\u0131 %0.5-1 oran\u0131nda artmaktad\u0131r. Bununla beraber 8. haftadan \u00f6nce radyasyona maruz kalan ve mikrosefali olan bebeklerde ise yap\u0131lan IQ testleri sonras\u0131 zeka gerili\u011fine rastlanmamaktad\u0131r.<\/p>\n

4. \u015eiddetli zeka gerili\u011fi:<\/strong> En hassas olunan d\u00f6nem 8-15 haftalard\u0131r. Atom bombas\u0131 sonras\u0131 yap\u0131lan g\u00f6zlemlerde gebeli\u011fin 8-15. haftalar\u0131nda radyasyona maruz kalanlardan do\u011fan bebeklerde al\u0131nan her rad ba\u015f\u0131na %0.4 oran\u0131nda \u015fiddetli zeka gerili\u011fi ortaya \u00e7\u0131kt\u0131\u011f\u0131 g\u00f6zlenmi\u015ftir. Al\u0131nan dozun 1 raddan daha d\u00fc\u015f\u00fck oldu\u011fu durumlarda ise buduruma hi\u00e7 rastlanmam\u0131\u015ft\u0131r. Hamileli\u011fin 7. haftas\u0131ndan \u00f6nce ya da 25. haftas\u0131ndan sonra radyasyona maruz kalanlarda ise \u015fiddetli zeka gerili\u011fi g\u00f6zlenmemi\u015ftir. Hamileli\u011fin 8-25 haftalar\u0131nda radyasyona maruz kalan bebeklerde her 100 rad i\u00e7in IQ d\u00fczeyinde 25 puanl\u0131k bir azalma g\u00f6zlenmektedir. Bu bebeklerde ya\u015famlar\u0131n\u0131n sonraki d\u00f6nemlerinde sara (epilepsi) hastal\u0131\u011f\u0131na da daha fazla rastlanmaktad\u0131r.<\/p>\n

5. Kanser:<\/strong> Radyasonun en korkulan etkisi uzun d\u00f6nemde ortaya \u00e7\u0131kan kanserlerdir.
